Appointments

A Scheduled appointment time-frame takes about 30 minutes per session. A session may consist of (1) CAC ID or up to (2) Dependent ID Cards. Please schedule multiple appointments based on the number of ID Cards needed.

All ID card seekers

All Adult individuals requesting ANY type of Military Identification Card will NEED one primary and one secondary form of Identification from the list below:

Primary form of identification required cannot be expired:

  • Current Military ID Card
  • Current State ID Card/State Drivers License
  • US Passport

Secondary forms of identification required:

  • Original Social Security Card
  • Original Birth Certificate

Cards are issued within 30 days of the expiration date on the front of the card.

Dependent ID cards

The sponsor must be present. If not, the dependent MUST have a signed DD Form 1172-2 (w/90 days of signature) signed by their sponsor & an official VO/SSM or a Power of Attorney that states to update DEERs. Call for further information.

Adding a spouse to DEERS

The sponsor needs to bring the following documents for their spouse: Official Marriage Certificate, and two forms of Identification. Photo ID (Drivers License or State ID not expired, Original Birth Certificate and the original Social Security Card.

Full time college students

All College students 21 to 23 years of age must bring a letter from their school registrar’s office stating that they are enrolled full time in an accredited college in pursuit of an Associate Degree or higher.

The letter will NEED to be on the schools letter head and include the students current semester/quarter dates and expected graduation date. Please call 951.655.3028 to request a sample of the letter if needed.

Age 23 to 26 Young Adult Tri-Care Programs

Member/Child will have to receive a Welcome Letter from Tri-Care by enrolling online at www.tricare.mil/mybenefit.

The military ID card will authorize the dependent Medical Coverage it will NOT authorize Commissary, Base Exchange or Military Warfare and Recreation (MWR) base privileges.
